Telephone Contact

Your telephone call is important to us, and we understand that you want to speak to us as quickly as possible.  If you are telephoning about a repeat prescription or information about test results, please call AFTER 11.00 am. 

 

Repeat Prescriptions

If you wish to order a repeat prescription you can use our dedicated Repeat Prescription line on 01305 217037 and leave a message.  Remember you must allow at least two working days before you need it (we need time to print the script, order the goods, dispense the order and check it).

On-Line Access to Medical Records

You can request the option to view your medical records on-line.  This is the coded version of your records and will include access to diagnosis codes/problems, test results, allergies and adverse reaction.

In order to protect your confidential records we ask you to come to the surgery to confirm your identity.  This is to ensure your records are only released to you.  This cannot be done over the telephone or by a family member on your behalf.  You will be asked to complete a form and we recommend you read the documents below as well.

Each application for access to records will be looked at and approved individually.  On the rare occasion where the GP is concerned that access may be harmful to a patient this will be discussed with that patient.

Proof of ID

Acceptable proof of ID should be photo ID such as a photo-driving licence or passport.  If you do not have any photo ID then please bring your birth certificate or a marriage, civil partnership or divorce certificate.  You will also need proof of address eg Council Tax Bill, utility bill, mortgage statement, bank statement, benefits statement or work permit/visa.  Proof of address must have been issued within the last 3 months.
